There's the very simple Polls Portlet. For more complex surveys, you can utilize Dynamic Data Lists. As they're more generic, you might have to work a bit on the presentation layer (display templates) but all in all they can do the job.

You can also try the RTG Survey portlet, which is available for purchase at Liferay's Marketplace.

You can use dynamic data list for some basic survey functionality.
